How Opioid Addiction Develops

Prescription and illicit opioids vary in potency and duration, but share core effects on opioid receptors. Opioids activate mu-opioid receptors, reducing pain and producing relaxation or euphoria while also slowing breathing. Repeated use can change reward and stress systems, leading to tolerance, craving, physical dependence, and opioid use disorder. After a period of abstinence, tolerance falls; returning to a previous dose can cause a fatal overdose.

Overdose and Other Dangers

Warning signs of opioid overdose include very slow or absent breathing, pinpoint pupils, blue or gray lips, choking or gurgling sounds, and inability to wake. Illicit fentanyl may be present in heroin, counterfeit pills, cocaine, or methamphetamine without the person’s knowledge. Combining opioids with alcohol, benzodiazepines, or other sedatives greatly increases respiratory-depression risk.

Withdrawal and Emergency Response

Opioid withdrawal can cause intense muscle aches, vomiting, diarrhea, anxiety, sweating, insomnia, and powerful cravings. It is usually not fatal by itself, but dehydration, pregnancy, other illnesses, and rapid return to use can create serious danger. For suspected overdose, call 911, give naloxone when available, provide rescue breathing if trained, and stay with the person because more than one naloxone dose may be needed.

What Treatment Can Include

Evidence-based care can include buprenorphine, methadone, or extended-release naltrexone; counseling and behavioral support; overdose education and naloxone; medical care; and treatment for co-occurring mental-health conditions. Medication for opioid use disorder can reduce withdrawal and craving and may continue for months or longer according to individual need.
